EVALUASI POTENSI INTERAKSI ANTIHIPERTENSI DAN DAMPAK TERHADAP OUTCOME KLINIS PADA PASIEN LANSIA DI INSTALANSI RAWAT INAP (IRNA) RSUP Dr. M JAMIL PADANG

Hipertensi merupakan penyakit degeneratif yang menyebabkan kematian
utama secara global sehingga disebut sebagai Silent Killer. Prevalensi hipertensi
tinggi pada lansia yang menyebabkan populasi ini rentan mengalami potensi
interaksi obat. Peluang potensi kejadian interaksi obat yang lebih tinggi pada
pasien yang mendapat polifarmasi. Tujuan dari penelitian ini adalah untuk
mengevaluasi interaksi obat antihipertensi dan dampak terhadap hasil klinis di
RSUP Dr. M. Djamil Padang. Penelitian non-observasional ini menggunakan
pendekatan cross-sectional dengan pengambilan data secara retrospektif dengan
jumlah sampel sebanyak 80 pasien. Data pasien dikumpulkan di bagian rekam
medis dari November 2022 hingga Januari 2023. Analisis persentase pasien yang
mengalami kejadian interaksi obat menggunakan cross tabulation pada uji Chi-
Square sedangkan analisis dampak terhadap hasil klinis pasien menggunakan
ANCOVA. Hasil penelitian menunjukkan pasien hipertensi lansia yang
mengalami interaksi obat sebanyak 44 pasien (55%) dengan jumlah 111 kasus.
Berdasarkan mekanisme interaksi obat yang tertinggi yaitu farmakodinamika 70
kasus (63,1%) dan farmakokinetika 41 kasus (36,9%). Berdasarkan tingkat
keparahan interaksi obat tertinggi yaitu moderate 71 kasus (64%), diikuti major 22
kasus (19,8%), dan minor 18 kasus (16,2%). Dampak interaksi obat terhadap
outcome klinis yaitu kejadian interaksi dan jumlah obat yang menyebabkan lama
rawat pasien semakin panjang dengan nilai p<0,05, sedangkan klasifikasi
hipertensi berpengaruh terhadap penurunan tekanan darah sistolik pasien dengan
nilai p<0,05. Namun, parameter tersebut tidak berpengaruh terhadap tekanan
darah diastolik dan denyut jantung pasien dengan nilai p>0,05.

Hypertension is a degenerative disease that causes major death globally, so
it is known as the Silent Killer. The high prevalence of hypertension in the elderly
makes this population susceptible to potential drug interactions. The chance of
potential drug interactions is higher in patients receiving polypharmacy. The
purpose of this study was to evaluate antihypertensive drug interactions and their
impact on clinical outcomes in Dr. M. Djamil Padang. This non-observational
study used a cross-sectional approach with retrospective data collection with a
total sample of 80 patients. Patient data was collected in the medical record
department from November 2022 to January 2023. Analysis of the percentage of
patients who experienced drug interaction events used cross tabulation in the Chi-
Square test while analysis of the impact on patient clinical outcomes used
ANCOVA. The results showed that there were 44 elderly hypertensive patients
who experienced drug interactions (55%) with a total of 111 cases. Based on the
mechanism of drug interaction, the highest were pharmacodynamics in 70 cases
(63.1%) and pharmacokinetics in 41 cases (36.9%). Based on the highest severity
of drug interactions, namely moderate 71 cases (64%), followed by major 22
cases (19.8%), and minor 18 cases (16.2%). The impact of drug interactions on
clinical outcomes, namely the incidence of interactions and the number of drugs
that cause longer patient length of stay with a p value <0.05, while the
classification of hypertension affects the decrease in patient's systolic blood
pressure with a p value <0.05. However, these parameters had no effect on the
patient's diastolic blood pressure and heart rate with a p-value> 0.05.
